Season four opens in Egg Harbor City, New Jersey, where a couple's once-cozy fixer-upper has devolved into a living nightmare. The lineup also features Browns Summit, North Carolina, a home where haunted activity is taking a toll on a family's health. A childhood home in Wabash, Indiana has become the site of paranormal disturbances since it was rebuilt after a mysterious fire. The former residence of a murderous mobster in Rock Island, Illinois is plagued by unexplained shadows and noises. In Watervleit, Michigan, a home is overwhelmed by aggressive activity that terrorizes its inhabitants. A 19th-century mansion in Buchanan, Virginia is steeped in dark secrets. And two haunted hotels—the Occidental Hotel in Buffalo, Wyoming and The Amargosa Hotel & Opera House in Death Valley, California—are rapidly losing business as terrified guests flee.

Steve and Amy investigate a family in Southern New Jersey who believe their house is haunted by an evil presence. Steve uncovers evidence from the late 1800s of a nearby health spa that was actually a sanatorium run by an allegedly insane doctor named Dr. Smith who believed he had found the fountain of youth. Steve also learns about a German immigrant, Louis Roesch, who arrived in town in the 1860s and was betrayed by his people because there was no civilization here. Meanwhile, Amy reports encountering a monstrous shadowman in the basement who tries to harm the living, including herself. Additionally, Amy claims to make contact with the previous owner, Louis Brunk, who died in the home, and with two of the Roesch family's six children.
